Code cleanup and minor refactoring to improve maintainability and the ability to internationalise the app.

Update README.md - add links (useful from Android browsers), update Disclaimer (from wiki version), plus other tweaks

Fix #16 - show registered USB sticks in the registration activity
Fix #14 - show successfully registered devices after CareLink login.

Removed new issue log reference (as per template change) and removed team members (as now mentioned in Wiki: Support)

Make only the log part of the UI scrolls.
